    We are in the process of implementing an innovative new grant-funded Chronic Care Case Management Program, similar to the cutting edge model that only three other states (North Carolina, Indiana and Georgia) are currently operating.

    The new Chronic Care Model we are implementing puts the Registered Nurse (BSN) in a more autonomous and pivotal position of educating the client in self-management where possible, and referring to physicians and clinics when necessary to avaoid unnecessary trips to hospital emergency rooms or residential placement.
