Expense Report

Enter your expenses in the appropriate spaces of the expense report; the form will automatically calculate your totals.  If you have mileage to report for the use of a personal automobile, enter your mileage in the row designated for mileage; the form will automatically calculate the reimbursement amount for mileage. In the section titled "Purpose of Trip or Site Visited", please enter the name of the institution that you visited. Once you complete the report, print it out. You can email it with the accompanying receipts as a PDF document to lcooperm@aacn.nche.edu, or fax to 202-887-8476, or mail to:
