Required Application Documents

To apply for CNL certification, the following forms must be submitted along with the appropriate application fee:

• CNL Certification Application
  

  • Online Application (for electronic submission)
  • Application Form (hard copy submission)

• Site Selection/Exam Administration

  • Instructions for Registering with a Testing Center
    If registering to sit for the exam at a site provided by the testing agency, you must first submit the application to CNC for review and approval. Following approval, CNC will notify the testing agency. The testing agency will send an authorization email  that will allow you select your exam site.
    

  • Instructions for Registering with a School of Nursing

• School of Nursing CNL Education Program Verification Form
  To be completed and signed by the Dean or equivalent – one form required per school. Institutions that have submitted the form are listed as an eligible institution.
  

• CNL Standards of Conduct

• CNL Education Documentation Form

• CNL Faculty Eligibility Documentation Form

• Change of Test Date/ Location Form

Note: All interested applicants should review the CNL Certification Guide and Application Checklist. It is the responsibility of the candidate to ensure submission of required documents to CNC by the school of nursing.  All required documents must be received by the published deadline.
