The National Committee for Quality Assurance (NCQA) defines a patient-centered medical home (PCMH) as "a model of care that strengthens the clinician-patient relationship by replacing episodic care with coordinated care and a long-term healing relationship. Each patient has a relationship with a primary care clinician who leads a team that takes collective responsibility for patient care, providing for the patient's health care needs and arranging for appropriate care with other qualified clinicians. The medical home model  is intended to result in more personalized, coordinated, effective and efficient care." Nurse Led Medical Homes are PCMHs in which a nurse practitioner oversees and coordinates patient care.

This Webinar will discuss establishing a successful nurse led medical home, without funding support from government sources or private foundations. Sean Lyon, MSN, APRN is the Patient-Centered Medical Home (PCMH) Project Director at Life Long Care, PLLC in New London, NH, the first nurse led medical home to achieve Level III recognition from the National Committee for Quality Assurance as a PCMH. This Webinar will focus on what a nurse should consider when choosing to establish a nurse led medical home including an introduction to the model and survey tools helpful to determining successful implementation.
